Hyde Park Central School District

It is the policy of the Hyde Park Central School District to provide a paper or electronic copy of the individualized education program (IEP) of a classified student to each regular education teacher, special education teacher, related service provider and other service providers who are responsible for the implementation of a student's IEP, prior to the implementation of the student's IEP, while respecting the confidentiality of the information included on the IEP. The Assistant Superintendent for Pupil Personnel Services or designee (such designee shall be a certified employee of the District with knowledge of student's disability and education program), shall ensure that the student's IEP is transmitted to appropriate teachers and service providers as described above, who are responsible for the implementation of a student's IEP, prior to the implementation of the student's IEP.

When providing a paper or electronic copy of an TEP to a student's teachers and service providers, the Assistant Superintendent for Pupil Personnel Services, or designee, shall also provide a student's teachers and service providers with a written statement explaining that the IEP is a confidential document and that redisclosure of the IEP can only be made under certain circumstances consistent with the Individuals with Disabilities Education Act, as amended, and the Family Educational Rights and Privacy Act, as amended.

The Assistant Superintendent for Pupil Personnel Services, or designee, shall also inform each teacher, teacher assistant, and any applicable support staff of their responsibility relating to the implementation of the IEP and the specific accommodations, modification, supports and/or services that must be provided for the student in accordance with the IEP.
