INTERSCHOLASTIC SPORTS PROCESS
Winter sports paperwork WILL NOT BE ACCEPTED AFTER November 10TH.
There are several important steps that must be taken in order to be eligible to try out. The steps are:
- Complete the Interscholastic Athletic Parent Consent, Eligibility and
Health Status Review, Parent-Interscholastic Sports Acknowledgement, Health History and Status Review Since Last Medical Examination and Parental Permission available in the Health Office. **Be sure that each of you signs in the appropriate spots.
- Return the packet to the Health Office as soon as possible.
- Screenings will be scheduled with Dr. Ritter or Pat Freeman, CFNP. The student will be seen by either for evaluation. If he/she has had a physical less than one year before the sport will start and we have a copy of it on file, the evaluation is a review with the student. Otherwise, the student will have a complete physical. If the student is found fit to play, the Interscholastic Athletic Parent Consent form is signed by the medical and nursing staff.
- The form is sent to Amy McArdle, Athletic Director, for review. She then signs it and gives it to the coach for the sport.
At this point, the student may participate in tryouts for the sport.
If you have any athletic questions, please call Amy McArdle, Athletic Director, at F.D.Roosevelt High School (229-4020 ext.1232). If you have any health office questions, please call the Haviland Middle School Health Office (229-4030 option 5).
